Before your visit
- New patients: please arrive 15 minutes early to complete paperwork.
- Bring a photo ID, your insurance card if you have one, and a list of your current medications and supplements.
- If you have recent lab results or records from another provider, bring them or let us know where to request them.
Need to cancel or reschedule?
Please let us know at least 24 hours ahead so we can offer the time to another patient.
For your privacy, please don't include medical details in online booking notes. We'll go over everything at your visit.
