What this website collects when you visit, and how we handle it.

This policy covers the Jordan River Primary Care website, www.jordanrivercare.com. It explains what the site collects when you visit and what we do with it.
Health information you share as a patient is protected by federal law (HIPAA). How we use and protect it is described in our Notice of Privacy Practices, which is available at the clinic. Just ask at the front desk.
The short version
- We don't sell or rent your information.
- We don't use advertising, tracking cookies or visitor analytics.
- The only personal information we receive through the site is what you choose to send us on the contact form.
Information you send us
Contact form. Our Contact page has a form for your name, how to reach you and a short message. The form is run by Google in the clinic's Google Workspace account, which is covered by a HIPAA business associate agreement with Google. We use what you send only to reply to you, and only clinic staff can see it.
Please keep messages short and don't include detailed health information. For anything about your care, call us or talk with us at your visit.
Email and social media. Regular email and social media messages aren't secure. Please don't use them to send health information.
Online booking. If you book an appointment online, the booking page is provided by a separate scheduling service. That service's own privacy terms apply to what you enter there.
Information collected automatically
Server logs. Like most websites, our hosting provider keeps standard logs of requests to the site. These include your IP address, browser type, the pages you open and the time. We use them only to keep the site secure and working. We don't use them to identify visitors.
Cookies. The public pages of this site don't set cookies. Staff sign-in uses cookies that are needed to keep staff signed in securely.
Services from other companies
To work well, some parts of the site load content from other companies. When your browser loads them, those companies may receive your IP address and basic browser details:
- Google Fonts (the site's typefaces) and Google Maps (the map on the Home and Contact pages)
- Google Forms (the contact form)
- jsDelivr (a service that delivers the site's layout and icon files)
- Our online booking provider, on the Book page
Links to Facebook, Instagram or other sites take you to services with their own privacy policies. They don't send us anything about you until you click.
Children
This website isn't directed to children under 13, and we don't knowingly collect information from them through the site.
Changes to this policy
If we change this policy, we'll post the new version here and update the date at the top.
